About The Position

The Referral Specialist assists patients by referring them to outside medical specialists or agencies at the request of AH attending physicians. The role contacts outside physicians or agencies, arranges appointments, coordinates the exchange of patient information, and works with patients to coordinate appointment locations and times. It also maintains a list of agencies that may provide financial aid or other assistance, prepares referral documentation and reports, screens applicants for a prescription program, and obtains and documents HMO authorizations.

This is a full-time position with a Monday–Thursday schedule from 8:00 to 6:30.

Requirements

Responsibilities

• Contact outside physicians or agencies by telephone, arrange patient appointments, and coordinate the exchange of patient information between AH providers and outside physicians or agencies.
• Work with patients to arrange and coordinate appointment locations and times.
• Maintain a list of agencies that patients requiring financial aid or other assistance can contact.
• Prepare referral logs, weekly reports, monthly reports, patient account documentation, and other necessary referral documentation.
• Screen applicants for the Putnam County Health Care Authority prescription program.
• Obtain authorizations from HMOs and document authorizations in patient medical records.
