About The Position

The Referral Specialist contributes to AIDS Healthcare Foundation (AHF) by conducting proactive outreach to clients, providing education, and assisting with the enrollment of the Ryan White eligibility program. This role involves working directly within the Healthcare Center to enroll newly diagnosed patients, patients lost to care, and clients currently in care by completing a screening eligibility assessment. The goal is to support the client’s ability to maintain or improve their health status and achieve their healthcare goals. The Referral Specialist enhances AHF's reputation by ensuring clients are appropriately screened for Ryan White Eligibility and other pertinent third-party benefits programs. This position requires meeting clients where they are, identifying strengths and opportunities for growth, and assisting with realistic goal setting. The role emphasizes collaboration and engagement to bring the essence of the Ryan White Care Act to the most vulnerable population. Eligibility assessments directly contribute to AHF’s success in meeting Ryan White contract deliverables and measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services. The Referral Specialist will be a passionate advocate for AHF's top initiatives, keeping AHF's Core Values and Mission top of mind. AHF operates in a dynamic environment, and this role requires adaptability to provide the best client and employee experience. The Referral Specialist will work as part of a close-knit team, collaborating with the Care Management team and Healthcare Center staff to ensure every Ryan White client receives the necessary support, Ryan White Eligibility assistance, and access to available resources.

Requirements

  • Two (2) years of verifiable experience in working with PLWH at an established agency can substitute on a year-for-year basis for an Associate degree.

Nice To Haves

  • Associate’s Degree/Bachelor’s Degree in social science or human services-related field preferred.

Responsibilities

  • Support the client’s ability to maintain better/improve their health status so they can work to achieve their healthcare goals.
  • Make all necessary efforts to ensure clients are appropriately screened for Ryan White Eligibility under all other pertinent third-party benefits programs.
  • Meet our clients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting.
  • Collaborate with the Care Management team and Healthcare Center staff so that every Ryan White client receive the support and Ryan White Eligibility assistance they require and access to available resources.
