About The Position

The Referral Specialist coordinates patient care between external providers and organizations, including physician offices, community service agencies, and hospital or emergency facilities. The role provides high-quality referrals and transitions by ensuring providers, facilities, and patients have the information and resources needed to coordinate care. It reviews clinical data to report medical necessity to insurance companies and establishes relationships and agreements among providers to support shared expectations for communication and care. The specialist also supports Referrals team members with shared tasks on a daily and weekly basis.

The position is full-time, Monday through Friday, 8:00 AM to 5:00 PM, at the Downtown Greer office. A remote option is mentioned, requiring home internet access and a computer.

Requirements

• High school diploma or equivalent.
• Proficient computer skills and typing.
• Proficiency with Microsoft Office and electronic medical record systems.
• At least 1 year of experience working in a medical office.
• Working knowledge of medical terminology and the ability to interpret physician progress notes and medical records.
• Exceptional communication skills with internal and external customers.
• Home internet access and a computer are required for the remote option.

Responsibilities

• Send outgoing referrals and diagnostic imaging orders to specialist offices and receive communications about processing those orders.
• Ensure patients are scheduled promptly in accordance with their diagnosis and level of care, and communicate appointment information to the patient, family, or caregiver.
• Follow up with specialist offices on pending referrals to obtain consult notes and medical records and coordinate continuation of care with the provider as needed.
• Promote excellent relationships between the practice and outside specialty offices.
• Serve patients, families, caregivers, external specialty offices, the community, and internal team members with grace and humility.
• Promote an authentic and transparent work environment.
• Support Referrals team members with shared daily and weekly tasks.
• Perform other tasks as assigned.
