About The Position

Requirements

• High school diploma or equivalent required
• Experience working in a healthcare or social services environment, preferably in a referral coordination or case management role.
• Strong knowledge of healthcare systems, insurance processes, and community resources.
• Excellent communication and interpersonal skills to interact effectively with diverse patient populations and healthcare professionals.
• Proficiency in using electronic health records (EHR) and referral management software.
• Strong organizational skills daily to manage multiple referrals efficiently and ensure no patient is overlooked.
• Communication skills are essential for explaining complex healthcare options clearly to patients and coordinating with various healthcare providers and agencies.
• Problem-solving skills help address barriers to care, such as insurance issues or transportation challenges, ensuring patients receive timely services.
• Technical proficiency with electronic health records and referral tracking systems enables accurate documentation and follow-up.
• Empathy and cultural competence are applied consistently to build trust and support patients from diverse backgrounds throughout the referral process.

Nice To Haves

• Associate’s degree or higher in healthcare, social work, or related field preferred.
• Certification in case management or patient navigation.
• Experience working with vulnerable populations, including elderly, disabled, or low-income patients.
• Familiarity with HIPAA regulations and patient privacy standards.

Responsibilities

• Evaluate patient needs and determine appropriate referral options based on clinical guidelines and patient preferences.
• Coordinate and schedule referrals with healthcare providers, specialists, and community resources to ensure timely access to services.
• Maintain detailed and accurate records of all referral activities, communications, and follow-ups in compliance with healthcare regulations.
• Communicate effectively with patients, families, healthcare teams, and external agencies to provide updates and resolve any referral-related issues.
• Monitor referral outcomes and follow up to ensure patients have successfully accessed recommended services and support.
• Educate patients about available healthcare and social services to empower informed decision-making.
• Collaborate with insurance companies to verify coverage and assist patients with authorization processes when necessary.
