Requirements

  • Experience working in a healthcare or social services environment, preferably in a referral coordination or case management role.
  • Strong knowledge of healthcare systems, insurance processes, and community resources.
  • Excellent communication and interpersonal skills to interact effectively with diverse patient populations and healthcare professionals.
  • Proficiency in using electronic health records (EHR) and referral management software.
  • Strong organizational skills daily to manage multiple referrals efficiently and ensure no patient is overlooked.
  • Communication skills are essential for explaining complex healthcare options clearly to patients and coordinating with various healthcare providers and agencies.
  • Problem-solving skills help address barriers to care, such as insurance issues or transportation challenges, ensuring patients receive timely services.
  • Technical proficiency with electronic health records and referral tracking systems enables accurate documentation and follow-up.
  • Empathy and cultural competence are applied consistently to build trust and support patients from diverse backgrounds throughout the referral process.

Nice To Haves

  • Associate’s degree or higher in healthcare, social work, or related field preferred.

Responsibilities
