About The Position

The Hospice Liaison/Admissions Nurse is responsible for coordinating hospice admissions from hospitals, healthcare facilities, physicians, and community referral sources. Working closely with patients, families, physicians, discharge planners, and hospice leadership, this role facilitates a smooth transition to hospice services while ensuring compliance with clinical, regulatory, and reimbursement requirements. The Hospice Liaison serves as a clinical consultant, educator, and customer service resource to referral sources and assists patients and families in understanding hospice services and determining the most appropriate level of care.

Requirements

  • RN license
  • Experience in hospice or home care
  • Knowledge of hospice regulations, reimbursement guidelines, and insurance requirements
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team

Responsibilities

  • Develop and coordinate the initial plan of care for patients admitted to Home Hospice or General Inpatient Hospice.
  • Collaborate with patients, families, physicians, case managers, and discharge planning personnel to obtain and review clinical information necessary for hospice admission.
  • Assess patient eligibility and coordinate appropriate hospice level-of-care placement.
  • Consult with Hospice Management regarding the appropriateness of General Inpatient Hospice admissions when needed.
  • Educate patients and families regarding hospice philosophy, services, benefits, and levels of care.
  • Obtain hospice admission orders from referring physicians and coordinate certification of terminal illness with Hospice Medical Directors.
  • Facilitate timely and seamless admissions while ensuring a positive patient and family experience.
  • Maintain current knowledge of hospice regulations, reimbursement guidelines, insurance requirements, and industry best practices.
  • Build and maintain strong relationships with referral sources, healthcare providers, and community partners.
  • Participate in quality improvement initiatives and perform other related duties as assigned.
