Requirements

Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
Ability to communicate effectively both verbally and in writing.
Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
Ability to work as a member in an interdisciplinary team.
Ability to apply decision making and analytical skills.
Ability to deal with others effectively.
Ability to role model conflict resolution strategies.
Physical ability to carry out the duties of the position.
Ability to prioritize and organize work.
Ability to operate related equipment.
Ability to teach and identify patient/resident learning needs.
Ability to lead patient/family group sessions.
Ability to provide clinical direction.

Responsibilities

Collaborate within interdisciplinary teams to deliver holistic, patient-centered care.
Assess and respond to patients’ physical, emotional, social, and spiritual needs.
Develop, implement, and evaluate individualized care plans.
Provide education and support to patients and their families.
Make appropriate referrals to internal and external resources.
Contribute to a culture of learning and support among healthcare professionals.

Conducts thorough patient/resident assessments through chart reviews, observations, interviews, and collaboration with peers.
Develops, implements, and evaluates nursing care plans based on evidence-based practices, prioritizing patient/resident needs.
Adheres to established policies, procedures, and PHC standards while providing high-quality nursing care.
Monitor patient/resident progress, evaluate outcomes, and adjust care plans as needed for optimal results.
Maintains accurate records of assessments, care plans, patient/resident status, and discharge plans using manual and computerized systems.
Identifies and address patient/resident and family learning needs, providing education and referrals as necessary.
Provides direction to staff, promote teamwork, and share clinical knowledge to enhance patient care.
Supports nursing research initiatives and contributes to the improvement of patient/resident care policies and procedures.
Advocates for patients/residents' rights and dignity, facilitates discharge plans, and collaborates with families and community resources.
Performs additional duties as assigned to support the healthcare team.
