About The Position

The Referral/Refill Clerk is an integral part of the Center’s health delivery system, focusing on coordinating all patient referrals at the request of providers. This role is responsible for coordinating referrals with patients, tracking received reports, and ensuring patients receive timely, high-quality, and efficient specialty care and support services through the referral system. The position requires familiarity with community resources to facilitate necessary services for patients at Omni Family Health. The role also includes ensuring HIPAA compliance, participating in QA/QI activities, and utilizing electronic health record systems.

Requirements

  • Thorough education or training in the basics of medical terminology and/or medical administration. Past work experience in the medical field will be considered in lieu of education or training in the basics of medical administration.
  • High school diploma or equivalent.
  • Ability to work with clinical staff and handle pressure from more than one provider.
  • Ability to work with frequent interruptions and manage multiple priorities.
  • Commitment to the concepts of preventive health care program and team approach to health care delivery.

Nice To Haves

  • Referral experience preferred.
  • Bilingual in English/Spanish preferred.

Responsibilities

  • Efficiently coordinate all patients’ referrals at the request of the providers.
  • Support patient health by appropriately scheduling and/or canceling all appointments, coordinating with patients and outside providers.
  • Determine and validate patient eligibility, validate patient demographic information, and pull records and file documentation.
  • Notify the patient of appointments and provide any necessary instructions.
  • Advocate for patient discounts with local vendors, including hospitals, laboratories, and individual medical providers, especially for sliding fee scale and self-pay patients.
  • Prepare authorizations necessary for outside patient care and health plan payment.
  • Proficiently track and record reports in the referral systems and electronic health record as they are received.
  • Follow up on pending referrals, including sending certified letters to non-compliant patients.
  • Cooperate with outside agencies to help patients apply for alternate resources when appropriate.
  • Promote and believe in OFH mission statement.
  • Relate to the public regardless of racial, ethnic, and economic status.
  • Maintain HIPAA compliance and treat all member information confidentially.
  • Ensure compliance with all local, state, and federal regulations.
  • Participate in QA/QI activities and contribute towards the overall performance improvement of the organization.
  • Learn and use the Electronic Health Record and Practice Electronic System and its components, including NextGen, PMS, QSI, and other electronic features.
  • Participate in the Patient Centered Health Home Model at Omni Family Health.
