About The Position

The Referral, Intake, and Eligibility Specialist is responsible for coordinating intake services for new and existing clients. The Referral, Intake, and Eligibility Specialist serves as a primary point of contact for incoming referrals from individuals seeking services and from community partners. The position is responsible for assessing immediate client needs, coordinating linkage to appropriate staff and services, maintaining knowledge of program regulations and procedures, and performing related work as required.

  • Ability to maintain accurate and timely documentation.
  • Ability to coordinate services with community partners and multidisciplinary teams.
  • Effectively communicate and build rapport with clients, colleagues, and other stakeholders.
  • Accurately determines program eligibility and ensures timely, complete, and compliant documentation.
  • Effectively manages high-volume intake and recertification processes while meeting deadlines and maintaining service quality.
  • Demonstrates empathy, active listening, and respectful management with clients from diverse backgrounds.
  • High School diploma or equivalent required.
  • Minimum of 1 year of experience working with persons living with HIV and/or health care training.

Responsibilities

  • Receives and processes incoming referrals from clients, healthcare providers, and community partner agencies.
  • Assesses immediate client needs and coordinates linkage to appropriate program staff and services.
  • Schedules and conducts eligibility interviews.
  • Obtains required documentation for Ryan White services via client, clinic, and community partners.
  • Schedules medical case management assessments as needed at time of eligibility appointment.
  • Provides overviews of agency programs and services to new clients.
  • Coordinates, monitors, and maintains Ryan White eligibility with other community agencies, reminding clients of upcoming eligibility appointments by phone and mail.
  • Assists clients with accessing insurance as well as other benefits programs.
  • Completes and submits VAMAP applications as needed for clients that are either uninsured or in need of secondary coverage (MPAP, ICAP, HIMAP, etc.).
  • Provides referrals and linkage to community resources and supportive services as appropriate.
  • Daily data entry of documentation into Provide and RedCap.
  • Participates in Quality Improvement activities.
  • Maintains chart documentation in accordance with agency and Ryan White standards.
  • Provides other office/program duties as needed.
