The Referral Coordinator is responsible for assisting providers with routine paperwork, such as Home Health forms and patient forms. This role involves the planning, implementation, and follow-up of patient referrals. The coordinator schedules patients for ancillary tests and appointments with outside providers, completes referral arrangements using appropriate forms, and maintains logs for tracking referrals outside the clinic. They also explain test or appointment preparation to patients, obtain insurance pre-authorizations or referral confirmations, and assist with Project Access eligibility, correspondence, and coordination with SRS Project Access staff. Communication with providers regarding scheduling, results, and follow-up is key, as is establishing and maintaining professional relationships with outside providers, labs, and medical facilities to ensure quality patient care and smooth referral operations. The coordinator handles patient phone calls and meetings to coordinate referrals and appointments, gathers necessary information for all involved parties, and assists with the flow of patient charts. They also retrieve test and consultation results, maintain and stock forms and supplies, and ensure patient confidentiality. Additionally, this role assists in developing and documenting protocols related to their job functions.
