About The Position

Ludlow Health is seeking an experienced Referral Coordinator to join our Federally Qualified Health Center (FQHC) in Ludlow, Vermont. This role oversees all diagnostic, specialty, and miscellaneous referrals, ensuring a seamless and positive patient experience. The Referral Coordinator greets and assists patients in person and by phone, performs patient registration and scheduling, manages documents, and supports front-desk functions. This position collaborates with clinical and administrative teams to deliver high-quality, community-focused care in a team-centered environment.

Requirements

Responsibilities

  • Process all diagnostic, specialty, and miscellaneous referral requests, ensuring appropriate supporting documentation is available.
  • Obtain authorization for referrals and schedule appointments for patients as required.
  • Maintain current knowledge of insurance prior authorization and referral requirements specific to payers.
  • Maintain current knowledge of North Star Health providers, service locations, and initiatives.
  • Collaborate with providers, patients, and payers to ensure timely completion of all referral requirements.
  • Track referrals to ensure patients are scheduled, attended appointments, and all necessary information is received from specialists.
  • Manage authorization worklist.
  • Complete all referrals in accordance with established policy and protocol.
  • Perform front-end administrative duties to meet patient needs efficiently.
  • Manage telephone calls professionally, including voicemail, patient portal, and answering service messages.
  • Schedule patient appointments following office protocols.
  • Triage phone calls for immediacy of problem and route messages appropriately.
  • Manage department inbox in an organized and systematic manner.
  • Manage appointment functions such as ticklers, reminders, portal scheduling, eligibility, wait lists, and patient reminder calls.
  • Handle all PHI according to North Star Health policy.
  • Greet patients in a professional, polite, prompt, helpful, friendly, positive, and welcoming manner.
  • Maintain professional composure in difficult situations, de-escalate, and assist when appropriate.
  • Demonstrate flexibility in work schedule and location based on organizational needs.
  • Show competency in all areas of registration and appointment workflows, including scanning documents, obtaining photos, updating patient information, and collecting co-pays and patient due balances.
  • Perform all other duties as assigned.
  • Adhere to policies and procedures during all activities.
