The Referral Coordinator is responsible for the referral coordination process at a Federally Qualified Health Center (FQHC), ensuring timely, accurate, and efficient patient referrals to external specialists and services. This position serves as a liaison between primary care providers, patients, specialists, and insurance providers. The Referral Coordinator plays a key role in supporting integrated, patient-centered care while ensuring compliance with FQHC and regulatory standards. This role manages the requests for patient medical records as well as the release of sed records. Risk adjustment chart reviews are managed by this role Inputs results of received cancer screening metrics from outside referral sources Partners with Chronic Care Management to communication enrolled patients admits and discharges from hospital or skilled nursing facilities.
