POSITION OBJECTIVE To deliver specialized, person-centered, and life skills interventions that empower individuals of all abilities to achieve meaningful independence, enhance quality of life, and progress toward individualized outcomes. JOB OVERVIEW The Recreational & Life Skills Specialist serves as a key contributor to the interdisciplinary care team, providing intentional, outcome-driven support to individuals receiving Supported Living services. This role extends beyond traditional direct care by applying therapeutic, evidence-informed approaches to skill-building, engagement, and personal development. The Recreational & Life Skills Specialist is responsible for facilitating structured and naturalistic learning opportunities that promote independence, self-determination, and community integration. Responsibilities include implementing individualized support plans, advancing life skill acquisition, supporting health and wellness routines, and ensuring accurate and timely documentation aligned with regulatory and organizational standards.

Requirements

  • Bachelor's/ Master’s degree in recreational therapy, or a related field
  • Certification as a Certified Therapeutic Recreation Specialist (CTRS)
  • Experience working with individuals with different abilities, ages, and backgrounds
  • Valid driver's license and reliable vehicle
  • No previous criminal record
  • Current First Aid, AED, and CPR certification (Offered by Sky Point, if needed)
  • Excellent organizational, interpersonal, and communication skills
  • Compassionate about the care and well-being of others
  • Ability to pass a background check and drug test
  • Smartphone with data and GPS capabilities
  • Basic computer skills and the ability to maintain electronic records
  • Applicants must be able to travel to the Fort Wayne office for the new-hire process, orientation, and, as needed, to stay compliant.

Responsibilities

  • Track client goals and objectives to support the completion of quarterly reports for the individuals served, and submit those reports to their supervisor.
  • Facilitate the development of daily living, social, and communication skills through structured and experiential learning.
  • Foster meaningful relationships that promote dignity, autonomy, and self-advocacy
  • Provide guidance and coaching to individuals and families to reinforce skill development across environments
  • Support individuals in accessing and engaging in community-based opportunities
  • Maintain timely, accurate, and compliant documentation, including progress notes and quarterly reports
  • Track data and outcomes to inform service delivery and demonstrate progress toward goals
  • Collaborate with supervisors and team members to ensure continuity and quality of care
  • Encourage individual to complete goals/objectives daily
  • Provide for the individual’s daily care that may include, but is not limited to:
  • Assist with bathing, dressing, and grooming
  • Assist with dining needs
  • Assist with transferring and ambulation
  • Teach/model/encourage communication/social skills
  • Offer emotional support to the individual and work with the family
  • Must follow Universal Precautions techniques in accordance with all agency policies and procedures, including the use of personal protective equipment
  • Maintains an environment that is healthy, safe, peaceful, and comfortable
  • Notifies the team and supervisor of any health concerns or needs for the individual supported
  • Follows all policies and procedures regarding safety in all environments
  • Transports individuals
  • Other tasks as assigned
