About The Position

Provide high intensity rehabilitation programming with a focus on neurological rehabilitation for outpatients post-discharge from hospital. Assess patient’s recreation and leisure needs, develop and deliver therapeutic programming and interventions and evaluate progress to enhance the client’s level of independence and safe functioning. Under the general direction of the Coordinator, Outpatient Neurological Rehabilitation and Outpatient Brain Injury Rehabilitation.

Requirements

Minimum of a two (2) year Diploma in Therapeutic Recreation, from an accredited college.
Registration with Therapeutic Recreation Ontario (TRO) is preferred.
Completion of the CorHealth’s Interprofessional Stroke Core Competency Framework or willingness to complete within one (1) month of employment is required.
Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
Experience working with patients who have had a stroke or other neurological and chronic conditions or muskuloskeletal disorders, in a rehabilitation or community setting is required.
Demonstrated advanced knowledge of neurological impairments (i.e. nature of injury, assessment, treatment, use of assistive technology, etc.).
Demonstrated knowledge of Community Agencies, Ministry of Health and other health and social services and available rehabilitation resources.
Demonstrated knowledge of Medical Terminology.
Demonstrated excellent judgment and proven analytical skills.
Demonstrated ability to mentor therapy assistants.
Demonstrated knowledge and understanding of the Ontario Hospital Association (OHA)/Ontario Medical Association’s (OMA) Communicable Disease Surveillance Protocols.
Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
Demonstrated superior interpersonal and communication skills, both written and verbal.
Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
Demonstrated discretion and maturity when handling confidential information.
Demonstrated commitment to the safety of co-workers and patients.
Successful Vulnerable Sector Check is required.
Demonstrated ability to work flexible hours as required.

Nice To Haves

Registration with Therapeutic Recreation Ontario (TRO) is preferred.
Experience with the administration of standardized assessments developed for neurological populations is preferred.
Experience working within an interdisciplinary team setting is preferred.

Responsibilities

Support implementation and adoption of CorHealth Ontario’s Model of Care for Community Stroke Rehabilitation.
Assess patient’s recreation/leisure needs using standardized assessments for neurological populations.
Develop a treatment plan to meet the needs of the patient based on assessments, patient interests, and objectives of therapy.
Observe, analyze, and record patients' participation, reactions, and progress during treatment sessions; modify treatment programs as required.
Engage the client in therapeutic leisure/interest activities in order to improve overall level of functioning and independence.
Recommend strategies, activity modifications, or adaptive equipment to compensate for residual difficulties, and/or increase safety and independence.
Assist the client and team in identifying appropriate community services to support reintegration and discharge from outpatient services.
Participate in or support a virtual model of therapeutic care as required.
Conduct home visits or support community outing(s) with the patient to address recreation and/or community reintegration goals as required.
Participate in the service’s discharge planning process.
Abide by the professional standards of practice established by Therapeutic Recreation Ontario (TRO).
Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
Educate and promote health, safety and wellness in the work place.
Represent the department or program on various committees and in meetings as required.
Perform other duties as required.
