About The Position

Liberation Programs is one of Fairfield County's leading behavioral health organizations specializing in treatment for all types of substance use disorders including alcohol, opiates, depressants and stimulants. Our goal is to help individuals and families overcome addiction in order to restore their lives and ultimately strengthen our communities. Liberation provides services for youth, adults and families that include two inpatient treatment programs, outpatient and intensive outpatient services, health education for older adults and people living with HIV/AIDS and other chronic illnesses, treatment and resources for adolescents and their families, education and prevention efforts in the community, and permanent supportive housing for families. In operation since 1971, the agency has sites in Greenwich, Stamford, Norwalk and Bridgeport and served more than 2,400 individuals in our last fiscal year. Our Mission: Liberation Programs' mission is to provide prevention, treatment and recovery services to help individuals and their families impacted by substance use and mental health conditions to foster hope and maintain wellness. Liberation Programs, Inc. is currently seeking a Recoveree Navigator to work at our Outpatient Treatment Facility in Stamford, CT. Job Purpose/Function: The Recovery Navigator will be responsible for answering calls generated from our toll-free number from our Wellness Resources Van as well as handle data entry for tracking CSSD clients. The Recovery navigator will also navigate, guide and onboard clients to the adequate program in support of their treatment and recovery process. The Recovery Navigator will also be a team player and a key member of our recovery team by being welcoming, empathetic and motivated to help others. Communicating with our clinical staff will also be essential with care navigation for recoverees.

Requirements

High School Diploma or G.E.D.
Good technical/computer skills; MS Office
Experience working with Electronic Health Records a plus
Able to comfortably and professionally interact with cross-cultural and diverse populations and demographics.
Experience working in a fast-paced environment
Self-Starter

Responsibilities

Retrieve and enter data in the electronic health record (EHR) system; scan medical and clinical paperwork into the EHR system
Enter data for CSSD client tracking
Handle large volume of calls
Review and communication insurance information and eligibility
Act as liaison between medical and clinical staff
Provide excellent customer service
Coordinate new recoveree onboarding
Schedule intakes
Track all new recoverees calling via resource number
Handle data entry to track results for syringe exchange and Narcan distribution
Ensure timely referrals to clinical staff
Additional duties as required, i.e. data entry in other systems
