About The Position

The Center, a community non-profit established in 1972, serves the San Diego LGBTQ community with targeted programs and services. Its mission is to enhance and sustain the health and well-being of the lesbian, gay, bisexual, queer, transgender, nonbinary, immigrant, and HIV communities. The Center aims to create a welcoming, valued, and supported environment for everyone. It is sex-positive and provides advocacy, critical health, and social support services, guided by values of equity, social justice, and client-centered, trauma-informed care. With over 75 staff across five facilities, The Center also operates the Sunburst Youth Housing Project, a 23-unit permanent supportive housing program for transitional-age youth, focusing on LGBTQ youth who may experience severe mental illness, HIV, or substance abuse issues. This position is located in the East Village neighborhood of San Diego, near San Diego City College.

Requirements

  • 1 year of experience providing case management and substance use recovery support services to people experiencing homelessness or at risk of homelessness.
  • Fluency in English required.
  • Must be comfortable working in PC/Windows-based environments.
  • Must have strong proficiency with Microsoft Office.
  • Demonstrated ability to manage multiple tasks with shifting priorities.
  • Must demonstrate competence in using strength-based, trauma-informed, and Motivational Interviewing Techniques.
  • Reliable transportation, a valid driver's license, and auto insurance.

Nice To Haves

  • Bachelor's degree in a social service-related field and/or Registered Addiction Counselor credential.
  • Fluency in Spanish.
  • 2 or more years of experience providing case management and substance use recovery support services to people experiencing homelessness or at risk of homelessness.
  • 1 or more years of experience providing case supportive services to people living with severe mental illness and/or substance use disorders.
  • Knowledgeable about current issues related to the LGBTQIA+ community and people experiencing homelessness.

Responsibilities

  • Conduct intakes, assessments, and evaluations with clients.
  • Provide links to mental health services, substance use recovery services, facilitates access to detox and treatment programs responsive to each participants' unique needs, educational services, career development services, and other relevant resources.
  • Provide ongoing case management, substance use recovery support, crisis intervention, conflict resolution, and stabilization services to participants enrolled in the program.
  • Collaborate with internal and external providers to support client care plans.
  • Assist clients with adherence to housing quality standards and attend housing unit inspections.
  • Organize community building events and facilitate workshops that promote client well-being and self-sufficiency.
  • Complete individualized service plans and self-sufficiency tools.
  • Maintain accurate documentation of client's goals and needs, case management plans, and service entries into internal data systems.
  • Enter program data and outcomes into the region's Homeless Management Information System (HMIS) to adhere to grant and contract compliance.
  • Document thorough case notes that accurately reflect client progress toward individualized case management goals.
  • Assist in the completion of monthly narrative reports for funder reports.
