Recovery Focused Case Manager
About The Position
The Center, a community non-profit established in 1972, is dedicated to enhancing the health and well-being of the San Diego LGBTQ community and related populations through targeted programs and services. With a focus on equity, social justice, and client-centered, trauma-informed care, The Center operates five facilities across San Diego County and employs over 75 staff. The Sunburst Youth Housing Project, opened in 2006, is a 23-unit permanent supportive housing program in the East Village neighborhood of San Diego, serving vulnerable transitional-age youth, with a focus on LGBTQ youth, some of whom experience severe mental illness, HIV, and/or substance abuse issues. The Recovery Focused Case Manager will work within this project, reporting to the Director of Youth Housing Services.
Requirements
- 1 year of experience providing case management and substance use recovery support services to people experiencing homelessness or at risk of homelessness.
- Fluency in English required.
- Must be comfortable working in PC/Windows-based environments.
- Must have strong proficiency with Microsoft Office.
- Demonstrated ability to manage multiple tasks with shifting priorities.
- Must demonstrate competence in using strength-based, trauma-informed, and Motivational Interviewing Techniques.
- Reliable transportation, a valid driver's license, and auto insurance.
Nice To Haves
- Bachelor's degree in a social service-related field and/or Registered Addiction Counselor credential.
- Fluency in Spanish.
- 2 or more years of experience providing case management and substance use recovery support services to people experiencing homelessness or at risk of homelessness.
- 1 or more years of experience providing case supportive services to people living with severe mental illness and/or substance use disorders.
- Knowledgeable about current issues related to the LGBTQIA+ community and people experiencing homelessness.
Responsibilities
- Provide case planning, substance use recovery support, and individualized supportive services to participants of The Center's Sunburst Youth Housing Project.
- Conduct assessments, organize community building events, facilitate workshops, and provide crisis support.
- Work collaboratively within a two-person case manager team in a trauma-informed and goal-oriented way to help program participants achieve housing stability and self-sufficiency.
- Conduct intakes, assessments, and evaluations with clients.
- Provide links to mental health services, substance use recovery services, detox and treatment programs, educational services, career development services, and other relevant resources.
- Provide ongoing case management, substance use recovery support, crisis intervention, conflict resolution, and stabilization services.
- Collaborate with internal and external providers to support client care plans.
- Assist clients with adherence to housing quality standards and attend housing unit inspections.
- Organize community building events and facilitate workshops that promote client well-being and self-sufficiency.
- Complete individualized service plans and self-sufficiency tools.
- Maintain accurate documentation of client's goals and needs, case management plans, and service entries into internal data systems.
- Enter program data and outcomes into the region's Homeless Management Information System (HMIS) to adhere to grant and contract compliance.
- Document thorough case notes that accurately reflect client progress toward individualized case management goals.
- Assist in the completion of monthly narrative reports for funder reports.
