The Whole Health Clinic has experienced significant growth with over 1,200 enrolled members and more than 3,000 appointments completed monthly. Each AHCCCS-insured patient is assigned a Recovery Coordinator to support care coordination, but current caseloads have become too large to maintain high-quality services while meeting all state compliance standards. Our AHCCCS contracts require Recovery Coordinators to fulfill extensive service delivery requirements including appointment and access standards, structured referral and intake processes, outreach and engagement, assessment and service planning, SMI eligibility determination, court-ordered treatment coordination, assistance with special populations, out-of-home placement coordination, and crisis management—all in addition to daily patient contact. Despite demonstrating high-quality outcomes and strong productivity, our Recovery Coordinators are overwhelmed. Since we cannot turn away AHCCCS referrals, an additional Recovery Coordinator position is essential to ensure appropriate member support, maintain contractual compliance, and reduce staff burnout and turnover. This position serves on an Adult Recovery Team (ART) assigned to each patient and acts as the primary point of contact for all ART participants to facilitate meetings and follow-up recommendations to ensure continuity of care and service delivery. This position assists facilitating a patient focused approach to member care providing personal assistance and skills training and other supportive services, to assist patients in improving their behavioral health and wellness. Assists patients in finding and securing personal, organizational, and community resources.
