About The Position

Educates members on health issues, including nutrition, diet and exercise. Also, provides linkage to health-related community resources, including smoking cessation programs. The Health Educator combines an understanding of wellness and overall health issues with knowledge to assist with linkage to appropriate services in the community.

Requirements

  • Proof of valid California driver’s license and auto insurance, as well as proof of education are required.
  • Medi-Cal Peer Support Specialist Certification required or must be obtained within the first 90 days of employment.
  • One year acute care experience and community clinic experience preferred.
  • Knowledge and experience working with the severely and persistently mentally ill population.
  • Intermediate level PC skills required.
  • Excellent organizational skills, interpersonal skills, computer skills and communication skills are a must.
  • Ability to effectively prioritize and manage time.
  • Requires extensive sitting with periodic standing and walking.
  • May be required to lift and/or move up to 20 pounds.
  • Requires significant use of computer, tablet, phone and/or general office equipment.
  • Needs adequate visual acuity, ability to grasp and handle objects.
  • Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone.
  • Will require off-site travel 20% (local and out of state) - meeting, county and/or client needs.
  • Will use personal car or vehicle as required for travel and/or transportation.
  • Will travel for meetings, training, etc. as needed.

  • Prior behavioral health experience preferred.

Responsibilities

  • Provides linkage services for members with physical, dental or mental health care needs.
  • Under direction and supervision of the Program Director, may provide health education and community linkage.
  • Establishes and maintains working relationships with community resources to assist in linkage.
  • Develops and teaches Wellness related classes and groups.
  • Functions as a resource to members and community resources and staff.
  • Maintain and ensures HIPAA, ethical standards and professional boundaries with all staff and clients.
  • Other duties as assigned or necessary to support the program and/or the company.
  • Provide the best customer care possible.
  • Identify and build upon the strengths of clients, coworkers, and the communities we serve.
  • Support clients’ steps towards Recovery and Wellness.
  • Create an organizational culture that respects and celebrates the diversity of our clients.
  • Value learning as an ongoing process that enables us to better service our clients and establishes our leadership in the industry.
