About The Position

Centurion is the provider of comprehensive services to the Maryland Department of Public Safety and Correctional Services. This full-time Discharge Planner (Social Worker – Re-Entry) position is part of the re-entry team at Baltimore Central Booking and Intake Center (BCBIC) in Baltimore, Maryland. The role involves completing discharge planning for patients referred to the multidisciplinary team who require community resources upon release. The Discharge Planner is responsible for ensuring timely and appropriate discharge plans by assessing post-release needs and providing necessary services. This individual will also act as a liaison between correctional healthcare providers and community agencies, collaborating with all relevant parties for release planning. Maintaining accurate and timely records related to release planning, including active files of available community resources, is also a key aspect of the role.

Requirements

  • Bachelor’s Degree in Psychology, Social Work, Counseling, or related field from an accredited educational institution
  • Must maintain current tuberculosis documentation
  • Must maintain active CPR certification (BLS)
  • Ability to obtain a security clearance, to include drug screen and criminal background check

Nice To Haves

  • Master’s Degree preferred
  • Minimum of two years’ experience in release planning preferred
  • Prior experience in working in a correctional healthcare program, criminal justice, or forensics preferred

Responsibilities

  • Complete discharge planning for patients referred to the multidisciplinary team based on the determination that community resources will be required at the time of release
  • Ensure that discharge planning is completed within the time period required by the client for the type of referral
  • Accomplish timely and appropriate discharge plans through assessment of the patient’s post-release needs and the provision of services necessary to meet those needs
  • Act as a liaison between correctional healthcare providers and community agencies
  • Collaborate and coordinate with all applicable parties (social work, security, etc) for release planning
  • Maintain accurate and timely records related to release planning, including active files of available community resources to facilitate the release planning
