About The Position

Northwest Specialty Hospital, an extension of Northern Nevada Medical Center, is expanding its services to a new building located at 6225 Sharlands Ave. in Northwest Reno. The facility will offer acute rehabilitation and adult behavioral health services. Our acute rehabilitation services include occupational, speech, and physical therapy for adults recovering from stroke, injury, or another disabling condition. We offer advanced treatment for a wide range of conditions including stroke, brain injury, amputation, neurological disorders, major multiple traumas and spinal cord and back injuries. All patients will experience private rooms, have access to a newly renovated therapy gym, and personalized care from our qualified staff. The adult behavioral health services offered is an expansion to our current 50 and older program at NNMC. The new program will offer behavioral health services to all adults 18 years and older. The department is managed through a multidisciplinary team including physicians, nurses, CNAs, social workers, and therapists. Patients will experience an individualized care plan, inclusive of individual and family therapy, activities, medication management, and a 24-hour secure, compassionate environment. This Social Worker/Discharge Planner opportunity is part time at 24 hours per week, offers full benefits and a convenient schedule. This dynamic individual will provide services to patients in the acute hospital setting. May also provide social services to patients in the sub-acute geropsychiatric unit as needed. Collaborates with the RN Case Manager in providing discharge planning services to patients. Is a resource for patients who require post acute care services, and psychosocial support during their stay in the hospital.

Requirements

  • Masters Social Work (MSW) preferred, BSW required.
  • Current state Social Worker licensure.
  • 3 or more years of previous experience in all aspects of social service and discharge planning preferably in an acute care setting.

Responsibilities

  • Demonstrates ability, and is responsible to perform psychosocial assessments on Medicare, Medicaid and any high-risk patients to determine social service and possible discharge planning needs. Documentation of assessments and interventions is timely and meets current practice standards and policies.
  • Maintains current knowledge of resources available within the community; maintains supply of resource materials to be distributed to patients when needed. Is able to obtain other resources as needed.
  • Communicates with family members and caretakers regarding the needs of the patient and anticipated plans. Involves patient and family in the plan of care.
