About The Position

The knowledge and skills needed for the Community Health Worker position are typically gained through a combination of lived experience with homelessness or incarceration AND navigating associated service delivery systems. For example: parole, probation, community clinics AND six months experience providing peer support and mentorship to individuals.

Requirements

Valid CA Class C Driver License or access to reliable transportation to carry out job-related essential functions
High school diploma or GED.
Proficiency in Microsoft Word, Excel, and Internet.
If in recovery, a minimum of three (3) years of being drug and alcohol free is required.
Neat and professional appearance.
Attention to detail and punctuality are required
Ability to communicate effectively, both written and orally required.
TB test, CPR and First Aid certification required.

Nice To Haves

A history of incarceration or homelessness is desirable
Prior experience working with currently and formerly incarcerated individuals, and/or homeless individuals is desirable
Prior experience as a health navigator, peer support worker, outreach worker or Promotora is desirable
Ability to speak Spanish, or other languages is desirable
Bachelor's Degree from an accredited college/university desired.

Comfortable working with diverse populations including LGBT, formerly incarcerated and mentally ill individuals

Able to work flexible job hours
Willingness to work in various environments, including jail settings, street outreach, home visits, homeless encampment
Attendance in all mandatory program and division meetings and trainings, as assigned by his/her supervisor and division director.
Maintain appropriate boundaries; and adhere to SSG's Code of Ethics and HOPICS Core Values.
Represent the Agency in a professional manner at meetings and community events.
Maintain files/records on client services in compliance with HIPAA, 42 CFR Part 2 and other funding requirements for audit purposes.
Regular attendance required.
Other duties as needed.
Perform other duties as assigned overtime, holiday, or weekend work may be required.
