Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. Requirements

Current Pennsylvania RN license.
Minimum of two (2) years of home health experience with progressive responsibilities.
OASIS certification is recommended; if not currently certified, must obtain within one year.
Knowledge of agency policies, federal and state regulations.
Strong interpersonal, communication, and organizational skills with evidence of leadership activities.
Proficient in home health computer documentation systems.
Reliable transportation with a valid driver’s license and insurance as required by state law.

Nice To Haves

OASIS certification is recommended; if not currently certified, must obtain within one year.

Responsibilities

Overseeing quality measures and data to ensure that submitted information aligns with OASIS guidance and regulatory requirements.
Conduct reviews of OASIS assessments in accordance with best practice guidelines.
Evaluate quality and process measure outcomes using advanced data mining software.
Improve ancillary services by analyzing assessments to foster positive patient outcomes.
Detect inconsistencies and errors in clinical documentation, reporting findings to the Quality/Education teams.
Take part in Quarterly Quality Record Reviews.
Work alongside the education team to provide quarterly OASIS training.
Participate in quality and rehospitalization meetings, assessing clinician scorecards and formulating corrective action plans as needed.
Perform audits to ensure adherence to all regulations governing home health and the appropriate use of services.
