About The Position

Assists with practice transformation efforts across primary care offices. Maintains knowledge and proactively identifies new or changing quality metrics and initiatives to be a resource for information and interpretation for SMC. Responsible for accurate and timely reporting of quality data. Performs extensive medical reviews and chart preparation to ensure appropriate documentation, compliance with quality metrics, and care coordination for patients.

Requirements

  • High school diploma or GED/HSE accredited by the State Board of Education.
  • One year experience in patient care.
  • Proficient in Microsoft applications and electronic medical records.

Nice To Haves

  • LPN experience with two years in healthcare with prior abstracting experience with knowledge of quality initiatives.
