Responsibilities

• Monitors patients from intake to the facility and ensures continuity of care leading up to and following release into the community.
• Advocates for patient needs while acting as a liaison between multiple systems impacting offenders' lives.
• Provides a proactive and collaborative approach in serving the patient population in a correctional environment.
• Oversees daily care management case calls and site log, including infirmary placement, COVID monitoring, Behavioral Health patients, hunger strikes, and crisis management.
