About The Position

The Quality Assurance Investigator at The Arc Ontario is responsible for conducting thorough, timely investigations into incidents reported to OPWDD or the Justice Center, ensuring all findings, documentation, and follow‑up actions meet regulatory standards. This role manages investigative records, gathers and preserves evidence, conducts interviews, collaborates with program staff, and recommends corrective actions to support agency compliance and service quality. The Investigator also assists with QA reporting, trend analysis, trainings, committee support, and various quality improvement activities while maintaining strict confidentiality and professionalism. Requirements

  • Must have a Bachelor's degree in human services, business administration or related field and over two (2) years of related job experience or an equivalent combination of education and experience.
  • Must have experience conducting trainings pertaining to abuse/neglect investigations.
  • Microsoft Word, Excel, Access, PowerPoint are necessary. Ability to master additional software (Therap, Relias, etc.) as needed.
  • Must have knowledge of investigative techniques.
  • Excellent interpersonal and communication skills are a necessity.
  • Must have an awareness of and ability to meet regulatory timelines.
  • Must have a valid NYS Driver's license.

Nice To Haves

  • Experience conducting investigations and a working knowledge of statutes, regulations and laws governing the care and treatment of persons with an intellectual disability is preferred.
  • Preference will be given to candidates with knowledge of OPWDD IRMA and Justice Center WSIR databases.

Responsibilities

  • Formulate, plan, process, and create timetables to conduct accurate, complete, and thorough investigation within OPWDD-specified timeframe.
  • Reviews the OPWDD Incident Review Management Application (IRMA) to ensure all required information is entered in a timely manner, keeps the team informed of new incidents as they occur in the database and enters information as needed.
  • Prepare for and conduct interviews and obtain sworn statements (when needed) on matters arising from investigations conducted, such as investigations for abuse, neglect, or other categories as defined by OPWDD and the NYS Justice Center.
