About The Position

The Qualified Professional (QP) coordinates, monitors, and delivers intensive, community-based wrap-around services to individuals with complex behavioral health and substance use needs. Operating under the the QP leads the multi-disciplinary care team, designs recovery-oriented intervention plans, and provides direct skill-building support. The QP ensures that all community-based services are clinically sound, culturally competent, and strictly compliant with state Medicaid guidelines.

Requirements

  • Must meet state-specific requirements to be designated as a Qualified Professional (QP) in behavioral health (typically defined by specific combinations of human services degrees and post-graduate experience).
  • Bachelor’s degree in a Human Services field (Psychology, Social Work, Counseling, etc.) with a minimum of [2 years] of post-degree accumulated behavioral health experience, OR Master’s degree in a Human Services field with a minimum of [1 year] of post-degree accumulated behavioral health experience.
  • Valid driver’s license, reliable personal transportation, and an excellent driving record for extensive community field travel.
  • Must successfully pass a comprehensive criminal background check, sex offender registry screening, and healthcare exclusion list verification (OIG).

Nice To Haves

  • Minimum of 2 years of experience working with high-acuity adults or youth in settings such as Assertive Community Treatment (ACT), Intensive In-Home (IIH), or community wrap-around programs.
  • Deep familiarity with writing Person-Centered Plans (PCPs) and utilizing Electronic Health Record (EHR) platforms for billing tracking.

Responsibilities

  • Lead the development, implementation, and regular review of the client's person-centered treatment plan (PCP) or comprehensive care plan.
  • Deliver face-to-face, structured skill-building interventions directly in the client’s natural environment to prevent out-of-home or restrictive placements.
  • Coordinate care across a multi-disciplinary team, managing case assignments and linking clients with medical, psychiatric, housing, and social services.
  • Develop proactive crisis plans with clients; provide active crisis monitoring, triage, and immediate coordination with emergency services when needed.
  • Manage, track, and ensure the clinical necessity of services billed under the 15-minute increment H2021 code structure.
  • Write audit-ready, comprehensive clinical progress notes within [24 hours] of service, clearly linking interventions to treatment plan goals and Medicaid compliance rules.
