About The Position

Easterseals PORT Health is seeking a full-time Community Support Team Qualified Professional to join their team in Surry and Yadkin Counties. This role involves providing services to individuals with mental health or substance use disorders who meet specific criteria. The position is community-based, with services including visits to clients in their homes. The Qualified Professional will participate in structured rehabilitative interventions focusing on areas such as housing, money management, daily activities, and personal wellness.

Requirements

In-depth knowledge of different therapeutic approaches and group techniques as well as their practical application in a community setting.
Ability to work independently.
Adept at problem solving and de-escalation in times of crisis.
Bachelor’s Degree from an approved accredited institution.
Minimum of two years’ experience working with the Adult Mental Health population.
A valid driver's license, current auto insurance and a good driving record.
Demonstrate high moral and ethical character, and be free of any substantiated charges of abuse or neglect on the NC Healthcare Registry.

Responsibilities

Provide services to individuals who have a mental health or substance use disorder and meet the two life domains.
Provide support services by participating in the provision of structured rehabilitative interventions to individuals.
Interventions may include housing and tenancy, money management, daily activities, and personal wellness.
Participate in the crisis phone rotation.
