About The Position
Carolina Outreach has been assisting children, families, and adults across North Carolina with their behavioral health needs since 2003. We are a group of highly trained behavioral health professionals dedicated to promoting the emotional and physical well-being of our community in a safe and respectful environment. We provide a comprehensive and integrative array of behavioral health services and supports for people of all ages with varying levels of need, utilizing an equitable and inclusive approach. We focus on helping our clients tap into and develop their inherent strengths with the goal of supporting the highest quality of life possible for all we serve. The primary purpose of this position is to provide Community Support Team Services to Mental Health and Substance Abuse consumers. This is an off-site job and its purpose is to provide these services in the community to enhance the consumer’s ability to function and achieve their goals.
Requirements
- Bachelor's degree from an accredited university in a Human Services field with 2 (two) years experience working with the population to be served.
- Bachelor’s Degree in a non-Human Service field with 4 years full-time, post-bachelor’s degree experience with the population served.
- Master’s degree in human services with 1 year experience working with population served.
- Valid driver’s license, auto insurance, and reliable transportation.
- Must complete the state mandated Community Support Team training within specified timeframes.
- Must be authorized to work in the United States.
Responsibilities
- Provide assistance and support to consumers in crisis situations.
- Service coordination activities, with or on behalf of a client.
- Psycho-educational and support activities for clients and their families and/or natural supports as appropriate.
- Rehabilitative and independent living skills.
- Symptom monitoring and management skills.
- Informing the client about benefits, community resources, and services.
- Assisting the client in accessing benefits and services.
- Arranging for the client to receive benefits and services.
- Monitoring the provision of services.
- Serve as a member of the client’s Community Support Team in the implementation of the consumer’s Person-Centered Plan.
- Assist in planning, scheduling and implementation of services for the clients.
- Work to implement service delivery with other staff and/or across agencies.
- Develop therapeutic interventions to work with consumers.
- Provide any written documentation as required, including treatment notes, goals, and plans for the medical record (e.g., Service notes, input into person-centered plan, etc.).
- Provide individual, family, and/or group therapy sessions.
- Maintain accurate and timely clinical documentation.
- Collaborate with care teams as needed.
- Maintain compliance with all clinical and regulatory standards.
- Participate in supervision (if applicable based on licensure level).
