About The Position

The Qualified Mental Health Specialist (QMHS) serves a critical case management and community-facing role within the Intensive Home-Based Therapy (IHBT) model. Working in close partnership with a Licensed Social Worker (LSW), the QMHS supports treatment implementation by ensuring assessments, service coordination, and community-based insights inform clinical decision-making. The Qualified Mental Health Specialist (QMHS) helps bridge therapy and real-world systems—supporting documentation, follow-through, and practical application of the LSW’s clinical direction while offering valuable perspective from the home, school, and community.

Requirements

  • Bachelor’s degree in social work, psychology, human services, or related field (required).
  • QMHS eligibility is determined as outlined per OH and Medicaid guidelines.
  • Strong organizational, communication, and follow-through skills.
  • Comfort working independently in community settings while collaborating closely with a clinical lead.
  • Reliable transportation and willingness to travel within the service area.

Nice To Haves

  • Masters preferred.
  • Experience in case management, community-based services, or youth/family support (preferred).

Responsibilities

  • Support completion, follow-up, and timely updates of required assessments, including Ohio CANS, under the clinical guidance of the LSW.
  • Assist families with navigating systems such as schools, probation, healthcare, and community resources.
  • Track service needs, referrals, and follow-through to reduce barriers to engagement and care.
  • Support families in accessing concrete resources (e.g., transportation, basic needs, community programs) aligned with treatment goals.
  • Maintain organized case management records and communicate updates to the LSW and supervisor.
  • Work alongside the LSW to observe, reinforce, and operationalize clinical insights gained through therapy sessions.
  • Support implementation of treatment goals through real-time coaching, modeling, and follow-up between sessions.
  • Provide the LSW with observational feedback from home, school, and community interactions that may inform treatment planning.
  • Reinforce therapeutic strategies and language introduced by the LSW, without independently delivering clinical therapy.
  • Serve as a consistent point of contact for families in community settings, strengthening engagement and continuity.
  • Offer insight into community dynamics, cultural context, environmental stressors, and strengths that impact family functioning.
  • Support coordination with schools, community partners, and natural supports as directed by the LSW.
  • Support in planning and execution of family department’s community events
  • Assist with school meetings, probation check-ins, and community interventions to support treatment goals.
  • Monitor safety, risk factors, and engagement challenges in day-to-day settings.
  • Support implementation of safety plans and escalation protocols as directed by the LSW.
  • Communicate concerns promptly to the LSW and supervisor in accordance with agency policy.
  • Participate in crisis support activities when required, within QMHS scope.
  • Complete timely, accurate, and Medicaid-compliant documentation related to case management and support services.
  • Ensure documentation aligns with treatment goals, Ohio CANS indicators, and IHBT requirements.
  • Support maintenance of complete and audit-ready case files.
  • Adhere to confidentiality, ethical standards, and IHBT program guidelines.
  • Participate in weekly team meetings, supervision, and case consultations.
  • Engage in required training related to IHBT, Ohio CANS, documentation, and community-based practice.
  • Maintain professionalism, cultural humility, and trauma-informed engagement at all times.
