Qualified Mental Health Professional (QMHP) - Visitation Specialist

Requirements
- Associate’s Degree, Bachelor’s preferred
- 1-2 years experience working with youth who have developmental or cognitive delays
- Must complete Autism Certification within first six months of employment
- Valid driver’s license
- 100/300/100 Auto Insurance Coverage
- Training and Travel willingness
- Successful completion of all required criminal background checks
- Must possess the skills to navigate and execute software programs (i.e Microsoft Office, Database, Electronic Health Record, Dashboard)
Responsibilities
- Utilize assessment tools and social history to identify the unique needs of the individual
- Plan, Lead and Execute daily treatment groups
- Utilize Trauma Informed Care approach and techniques in service delivery
- Ensure all documentation is accurate and timely when entered into the electronic health record
- Navigate the electronic health record to gather information needed to best serve individuals
- Assist individuals with skill development and activities based on level of functioning
- Collaborate with various program leaders to ensure continuity of care
- Develop group activities in accordance with the treatment needs identified in the Individual Service Plan.
- Ensure compliance of certification and program requirements through ongoing training and development
