About The Position

This position involves coordinating the development and implementation of client-based habilitative programming, such as individual habilitation plans (IHP), individual program plans (IPP), and individual behavior guidelines. The role also includes acting as chairperson for inter-disciplinary team meetings to coordinate comprehensive service delivery from various specialists (speech, medical, pharmacy, social services, psychology, psychiatry, vocational, educational). The professional will supervise assigned staff in delivering services, train staff in effective techniques, and ensure habilitative services and documentation meet requirements (e.g., Medicaid ICF/IID). This includes maintaining accurate records, reviewing programs for effectiveness and compliance, and initiating modifications as necessary. The role also involves conducting periodic rounds to ensure proper programming implementation and monitoring staff interactions and compliance with regulations. Additionally, the professional chairs team meetings for client habilitation plan reviews, facilitates discussions, develops and prioritizes client goals, and writes individual programs. Responsibilities extend to coordinating client home visits, transfers, and discharges, acting as relief residential care supervisor, assisting with client training, attending meetings and training, serving on committees, acting as a liaison with families and community staff, preparing reports, and assisting with youth admissions and releases.

Requirements

  • At least one year of experience working directly with persons with intellectual disability or other developmental disabilities
  • Doctor of Medicine or Osteopathy
  • Registered Nurse
  • Holds at least a bachelor’s degree in a professional human services category (e.g., Psychology, Education, Anthropology)
  • Job Skills: Direct Support/Direct Care
  • Final candidate selected will be required to undergo a criminal background check
  • Final applicant selected will be required to submit to urinalysis prior to the appointment to test for illegal drug use

Responsibilities

  • Coordinates overall development & implementation of client-based habilitative programming (example, individual habilitation plan (IHP); individual program plan (IPP); individual behavior guidelines)
  • Acts as chairperson of inter-disciplinary team meetings in order to coordinate comprehensive delivery of services (example, speech, medical, pharmacy, social services, psychology, psychiatry, vocational, educational) and model appropriate behavior/intervention to assigned caseload of 8-30 clients
  • Supervises assigned staff (example, residential care supervisors; hospital aide coordinators; therapeutic program workers; teachers; activity therapy specialists; juvenile correctional officers; social workers) in delivery of services
  • Trains staff in effective delivery of training techniques/methods
  • Ensures habilitative services & documentation meet requirements (example, Medicaid ICF/IID) & maintains accurate records of program implementation
  • Reviews programs to determine effectiveness/compliance & initiates modifications/corrective action if necessary
  • Conducts periodic rounds of cottages/modules/units/buildings or other active treatment locations to ensure proper implementation of programming & to monitor staff (example., therapeutic program workers) in proper documentation, appropriate client interactions & compliance with Medicaid ICF/IID regulations and standards
  • Acts as chairperson of team meetings to review, analyze and modify 30/90 day & annual reviews of client habilitation plans to include review of information prior to team meetings, chairing meetings, facilitation of discussion, development of goals and objectives for clients, prioritization of goals and objectives, proofreading minutes of meetings and writing of individual programs, changes to and/or modifications of client programs on ongoing basis and correction of deficiencies
  • Participates in mock surveys of facilities; recommends proposed policies and procedures
  • Coordinates client home visits, transfers (example, inter/intra facility) and discharges to ensure efficiency
  • Acts as relief residential care supervisor and/or assists therapeutic program workers in training clients
  • Attends meetings, training, workshops &seminars
  • Serves on committees and task forces
  • Acts as liaison with client families, guardians, case management staff and/or community-based staff
  • Prepares reports, forms and correspondence related to clients and staff assigned
  • Assists with coordination of youth admissions and releases
