About The Position

Cliffview Assisted Living is a 28-bed facility located in Kremmling, CO, in Grand County. The PCP/Nurse Aide/QMAP is responsible for providing basic nursing care to the 28 resident population within their Scope of Practice, maintaining a safe and clean patient care environment. This role is an integral part of the health care team, fostering healthy relationships with residents, families, and team members, and contributing to optimal patient care and outcomes.

Requirements

High school graduate or GED equivalent.
Basic Life Support – BLS (or within 60 days of hire date).

Nice To Haves

Licensed as a Certified Nurse Assistant in the State of Colorado.
Graduate from a State Certified Program for Nursing Assistants.
6 months prior experience as a CNA.

Responsibilities

Providing basic nursing care to residents within their Scope of Practice.
Maintaining a safe and clean patient care environment.
Participating as an integral member of the health care team.
Establishing and maintaining healthy relationships with residents, resident family members, and health care team members.
Communicating information that contributes to optimal patient care and the achievement of desired and defined patient outcomes.
