About The Position

The Assisted Living Nurse addresses the medical, social, psychosocial, and mental health needs of the neighbors to enhance their quality of life in the Assisted Living. This role involves assessing and monitoring neighbors' needs, completing required reports, training and mentoring AL QMAPs, overseeing medication administration, conducting medication cart and pass audits, assisting neighbors and families with mood and psychosocial needs, aiding with admission and discharge processes, providing accurate documentation, attending psychotropic drug review meetings, reviewing incidents, assessing falls and changes in condition, communicating with physicians and families, assisting with survey preparations and quality improvement initiatives, and making regular rounds on all neighbors. The nurse administers prescribed medications and treatments, safeguards drugs, and assumes responsibility for professional growth, maintaining confidentiality and knowledge of state and federal regulations. The role may also include developing policies and procedures and ensuring neighbor rights are followed. Additional duties may be assigned as needed.

Requirements

  • Registered Nurse with a current license to practice in the state of Colorado. LPN may be considered.
  • Patience needed to work with the needs and demands of neighbors.
  • Ability to enter data accurately.
  • Ability to think critically, be detail-oriented, work well with peers, handle multiple interruptions, and work on multiple tasks throughout the day.
  • Good writing and communication skills.
  • Ability to communicate well with neighbors/clients, their families, and other staff.
  • Must exhibit courteous, compassionate, and respectful treatment of neighbors/clients, families, and co-workers.
  • Ability to listen well and process information.
  • Knowledge of medical terminology.
  • Must be able to sit, stand, and walk 8 hours a day.
  • Must be able to lift and carry 0-50 pounds regularly and 50+ pounds occasionally with assistance.
  • Must be able to push/pull up to 250 pounds regularly using a wheelchair.
  • Sits, stands, bends, walks, reaches, lifts, keyboarding, pushing wheelchairs, and moves intermittently during working hours.
  • Employee is subject to physically ill neighbors as well as hostile and emotionally upset neighbors and/or families.

Nice To Haves

  • Healthcare experience in a geriatric setting preferred.
  • Experience with Medicare and Medicaid regulations.
  • Experience with long-term care, working with the elderly, and knowledge pertaining to dementia and related issues.

Responsibilities

  • Assessing and monitoring neighbors' medical, psychosocial, and mental health needs and reporting to proper agencies and individuals as necessary.
  • Completing required reports related to neighbor's medical, psychosocial, and mental health.
  • Training, mentoring, and assisting with performance management of AL QMAPs.
  • Overseeing medication administration of QMAP’s and providing necessary training.
  • Conducting medication cart audits at least quarterly and performing medication pass audits and chart reviews.
  • Serving and assisting neighbors and families with mood and psychosocial needs, specifically in the areas of family and/or resident resolution of issues.
  • Assisting with admission screening, admission processes, and discharge planning.
  • Providing accurate and timely documentation and updating care plans.
  • Attending psychotropic drug review meetings and presenting necessary information on neighbors.
  • Reviewing all incidents and at times responsible for incident reporting and abuse/neglect investigations.
  • Assessing neighbor falling a fall or a change in condition within 24 hours of a weekday event.
  • Communicating neighbors’ condition & significant issues with physicians, families, staff and Administrator as necessary.
  • Assisting the department in survey preparations and regulatory compliance and assisting the administrator with quality improvement initiatives.
  • Making regular rounds on all neighbors.
  • Administering prescribed medications and treatment in accordance with the Nurse Practice Act, with an understanding of the nursing implications (drug actions, interactions, adverse effects, etc.) in terms of patient diagnosis.
  • Safeguarding and administering drugs in accordance with the law and with established policies.
  • Assuming responsibility for personal and professional growth.
  • Maintaining confidentiality of all work-related information according to HIPAA regulations.
  • Obtaining necessary knowledge of State and Federal regulations.
  • Participating in developing policies and procedures pertaining to their job roles.
  • Assuring neighbors/clients' rights, privacy, and HIPAA regulations are followed.
  • May serve on Eben Ezer committees.
  • May need to assist with neighbors’ transportation on occasion.
  • Attending facility meetings and department meetings as instructed.
