Pulmonary Program Coordinator (RN)
About The Position
Garden Gate Health Care Facility is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate our Pulmonary Program. In this role, you'll provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs. Requirements
- Active New York State Registered Nurse (RN) license required
- Strong clinical assessment and care coordination skills
- Strong leadership, organizational, and communication skills
Nice To Haves
- Skilled nursing, rehabilitation, pulmonary, respiratory, or subacute experience preferred
- Experience with care planning and interdisciplinary collaboration preferred
Responsibilities
- Oversee and coordinate daily operations of the Pulmonary Program
- Coordinate individualized care plans for residents with pulmonary and respiratory needs
- Monitor residents with respiratory and pulmonary conditions
- Collaborate with physicians, respiratory providers, therapy, nursing, and interdisciplinary teams
- Assist with admissions, assessments, and care coordination for pulmonary residents
- Provide staff education on pulmonary care and clinical best practices
- Ensure compliance with regulatory and clinical standards
- Participate in quality improvement and program development initiatives
