About The Position

This position is anticipated to close on July 1, 2026, but applications will be accepted until position is filled and removed from the Delta County jobs website. This position works 4 or 5 days per week (0.8 or 1.0 FTE). Provide comprehensive, unbiased nursing care across Delta County Health Department’s public health programs with a focus on immunizations, communicable diseases, child health and supporting family planning services to ensure smooth clinic operations and high-quality community health services.

Requirements

  • Associate’s degree in nursing required
  • At least three (3) years of nursing experience in a Public Health setting, physician’s office, or community clinical setting preferred
  • Proficiency and experience in AthenaOne (electronic health record) is preferred.
  • Combination of education and experience could waive specific requirement.
  • Current and unrestricted Colorado Registered Nurse license
  • Current and unrestricted American Heart Association BLS certificate
  • Current and valid Colorado Driver’s License

Responsibilities

  • Conduct health assessments, gather histories, administer diagnostic tests, and provide nursing services such as vitals checks, immunizations, and medication administration.
  • As part of a larger strategic plan, build, maintain, conduct processes and outcome measures of supportive grant programs. Responsibility to adhere to grant reporting guidelines as assigned.
  • Develop individualized care plans based on client needs and goals, implement and re-evaluate these plans, and make referrals to other providers or resources as needed.
  • Educate clients on immunizations, manage vaccines, maintain records in the state immunization system, and oversee off-site clinics.
  • Conduct TB and communicable disease screenings, provide education, and manage treatment plans and performing epidemiological investigations in line with CDC and CDPHE guidelines. Educating the community on prevention measures, track cases, and maintain accurate records
  • Be able to provide support on sexual and reproductive health services, chart, check in patients and collecting and labeling lab samples, and education.
  • Perform care coordination, develop family care plans, and manage documentation for child health programs.
  • Collaborate with team members to support overall clinic operations, ensuring efficient and effective care across all service areas.
  • Offer counseling, education, and outreach to individuals, families, and communities on disease prevention, wellness, and available health programs.
  • Maintain accurate records in compliance with health department policies, including using electronic health records and following HIPAA standards.
  • Demonstrate competencies in the Incident Command System for public health emergencies (NIMS-700, ICS-100, ICS-200).
  • Work effectively with other health department staff across diverse programs and participate in health department and community committees, staff meetings, and educational programs.
  • Assist with program development and funding efforts.
  • Perform inventory management, facility preparation, and equipment sterilization.
  • Participate in epidemiological investigations and conduct community health surveys.
  • Market health department services and maintain professional relationships with community partners, CDPHE staff, and medical providers.
  • Flexibility to perform related duties as assigned, support disaster preparedness, work well with diverse cultures, and contribute to a cooperative team environment.
  • Should the Board of County Commissioners or another authorized representative issue a local disaster declaration in Delta County, all Delta County employees may be called upon to serve as disaster service workers.
