PHMC serves as both a direct service provider and an intermediary agent, managing large-scale contracts, government and philanthropic partnerships, and multidisciplinary initiatives. The National Nurse-Led Care Consortium (NNCC) is dedicated to advancing nurse-led models of care. The Public Health Project Coordinator is responsible for the day-to-day activities of one or more public health projects for NNCC, which may be funded by various sources. This role supports reporting responsibilities, coordinates project planning and execution, works with stakeholders, clients, and home visiting staff, and provides data entry assistance for tracking deliverables and monitoring performance measures. This position will primarily support the Family Advocacy and Integrated Resources (FAIR) project, a new initiative connecting families served by home visiting programs in Philadelphia to civil legal services and funds for concrete needs. The Coordinator will work closely with the FAIR Project Manager and Director of Nurse Advocacy and Resources, as well as the Fund Manager at PHMC, to conduct complex case navigation for clients and ensure timely assistance to families experiencing urgent needs. The position is located in Philadelphia, PA and reports to the FAIR Project Manager.
