About The Position

This position provides public health nursing services to promote and protect the health of Tribal members and the community. Responsibilities include health promotion, disease prevention, communicable disease control, chronic disease management, care coordination, community outreach, and collaboration with Tribal programs, healthcare providers, and community partners to improve individual and community health outcomes through culturally responsive and evidence-based practices.

Requirements

Associate Degree in Nursing (ADN) Registered Nurse (RN) license in the State of Oregon.
Minimum of three (3) years nursing experience, including two (2) years in a clinical or community health setting.
Bachelor of Science in Nursing (BSN) Registered Nurse (RN) license in the State of Oregon.
Knowledge of community and public health nursing principles, practices, and procedures.
Knowledge of disease prevention, health promotion, population health, and community-based health interventions.
Knowledge of population health assessment methods and strategies used to identify and address community health needs.
Knowledge of epidemiology principles and communicable disease surveillance, investigation, reporting, and control measures.
Knowledge of public health response activities, including case investigation, contact follow-up, outbreak response, and immunization practices.
Knowledge of Federal, state, Tribal, and local public health regulations, standards, and reporting requirements.
Experience in leadership - Inspires, motivates, and guides others toward accomplishing goals. Achieves desired results through effective people management.
Experience in conflict resolution - Influences others to build consensus and gain cooperation. Proactively resolves conflicts in a positive and constructive manner.
Experience in critical thinking – Identifies complex problems. Involves key parties, gathers pertinent data and considers various options in decision making process. Develops, evaluates and implements effective solutions.
Experience in communication and team building, leads effectively with excellent verbal and written communication. Delegates and initiates/manages cross-functional teams and multi-disciplinary projects.
Ability to coordinate care and collaborate effectively with interdisciplinary teams, tribal programs, community organizations, and public health partners.
Ability to established public health policies, procedures, guidelines, and applicable nurse practice act; exercise sound judgment within the scope of assessment.
BLS certification required.
Must possess a valid Driver’s License and be eligible to drive a GSA Vehicle.

Responsibilities

Integrates public health principles with clinical nursing practice to support program staff with adult and child health, communicable/reportable disease prevention and control, chronic disease management, and maternal and child health (prenatal and postpartum support), and health promotion needs across the lifespan.
Leads communicable disease reporting, case investigation, contact trace and treatment follow-up in accordance with public health requirements
Participates in the planning and evaluation of public health programs and services to assess effectiveness and support quality improvement.
Maintains accurate documentation and required reports in accordance with policy and regulatory requirements.
Maintains professional competency through continuing education and adherence to professional nursing standards.
Respond quickly and appropriately to situations that may arise.
Use critical thinking skills to adjust actions in order to provide patient centered care.
Adapt to rapidly changing situations.
Use effective written and oral communication skills to explain complex medical issues, work with team members and providers, and discuss difficult or sensitive issues.
Listen and understand complex information.
Be an effective member of a multi-disciplinary care team.
