About The Position

The Community Health Advocate Coordinator serves the Siletz Tribal population by providing care coordination, advocacy, outreach, and support services through the Siletz Community Health Clinic and its wraparound service programs. This position works collaboratively with patients, families, medical providers, behavioral health, dental, community resources, and multidisciplinary teams to improve access to care, promote wellness, address barriers to services, and support positive health outcomes.

Requirements

• Associate degree in Healthcare, Human Services, Behavioral Health, Public Health, Social Services, or a related field and Two (2) years of experience in healthcare, case management, patient advocacy, social services, community health, or related field required; OR an equivalent combination of education, training, and/or experience.
• Traditional Health Worker (THW) certification is required within six (6) months of hire.
• Knowledge of community health practices, patient advocacy, care coordination, and wraparound service delivery models.
• Knowledge of healthcare systems, community resources, referral processes, and social service programs that support patient wellness and continuity of care.
• Knowledge of culturally responsive care practices and the unique healthcare needs of Tribal and underserved populations.
• Knowledge of confidentiality standards, HIPAA regulations, and professional boundaries related to patient information and services.
• Knowledge of communication, conflict resolution, and relationship-building techniques with patients, families, providers, and community partners.
• Experience in operating Microsoft Word, PowerPoint and production of documents utilizing imported graphics.
• Public speaking skills.
• Demonstrated experience working with Tribal communities and/or Indigenous populations required.
• Strong understanding of culturally responsive and community-centered approaches to care and advocacy.
• Ability to coordinate and oversee the delivery of patient support services provided by Community Health Advocates, ensuring individuals are connected to healthcare, social services, and community resources in a timely and culturally responsive manner.
• Ability to provide guidance, and support to a team of Community Health Advocates, including prioritizing workflows, monitoring service delivery, and promoting professional development and accountability.
• Ability to build and maintain collaborative relationships with Tribal members, clinic staff, healthcare providers, community organizations, and multidisciplinary teams to strengthen coordination of care and community outreach efforts.
• Ability to maintain oversight of documentation, reporting, and program activities.
• Ability to communicate effectively both verbally and in writing, including facilitating team meetings, coordinating trainings, providing education, and representing the program through community outreach and partnership activities.

Responsibilities

• Coordinate and oversee the Community Health Advocate program and related wraparound service activities that support Tribal members and families served through the Siletz Community Health Clinic and Tribal community programs.
• Provide guidance, and support to Community Health Advocates, in service delivery.
• Coordinate outreach, engagement, and follow-up activities conducted by Community Health Advocates to improve access to care, continuity of services, and community wellness outcomes.
• Collaborate with Community Health Manager to strengthen coordination of care and support services for Tribal members.
• Support Community Health Advocates in identifying and addressing barriers that impact access to healthcare, social services, and treatment compliance.
• Monitor program documentation, charting, referrals, reporting, and data collection activities to ensure accuracy, timeliness, and compliance with clinic policies, grant requirements, and confidentiality standards.
• Coordinate community health education efforts, wellness initiatives, outreach events, and prevention activities in alignment with Tribal health priorities and community needs.
• Promote culturally responsive and trauma-informed practices while fostering positive relationships between the clinic, Tribal programs, community organizations, and Tribal members.
• Assist with program planning, quality improvement activities, policy implementation, and evaluation of Community Health Advocate services and outcomes.
