About The Position

This role ensures delivery of essential public health services in a professional and collaborative way, supporting the health and well-being of individuals, families, and the wider community. The position primarily provides Nurse-Family Partnership (NFP) home visiting services, along with other nursing and public health duties as needed.

Requirements

  • Bachelor's degree in nursing (BSN).
  • Ability to read procedural documents, write reports, and speak effectively in group settings.
  • Basic math skills for budgeting/program calculations.
  • Proficiency with word processing, spreadsheets, internet, program management, and database software.
  • Current Colorado RN license, liability insurance, and Colorado driver's license.

Responsibilities

  • Conducts NFP nurse home visits following program guidelines.
  • Attends required NFP training and meetings.
  • Provides referrals to community resources.
  • Delivers immunization services and participates in communicable disease reporting/investigation when required.
  • May supervise paraprofessional staff.
  • Maintains emergency preparedness certifications.
  • Ensures consistent, reliable attendance.
