About The Position

The Tulare County Health & Human Services Agency is currently recruiting for a Public Health Nurse (PHN) I/II. The desired candidate will have the ability to develop, implement, and interpret policies and procedures, analyze information, assess health statuses, and recommend appropriate courses of action. The PHNs are primarily oriented to the provision of community nursing services. These services focus on concepts of prevention, early intervention, communicable disease investigation, case management, and health education. Services may be delivered within the department, in homes, clinics, schools, and by liaison agreements with other agencies or organizations. These services include assessment of individual, family, or community health status; referral to appropriate organizations for treatment; demonstration and teaching of health concepts to individuals and groups; and mobilizing of community resources to provide for continuity of client care between acute and extended care facilities and the home. This recruitment will establish an employment list to fill immediate and future vacancies throughout the Agency’s Public Health and Child Welfare Services Division. The Public Health Branch offers a progressive and innovative working environment for employees while providing comprehensive health care in the community. The chosen candidate will work with public health programs with comprehensive case management for vulnerable populations. The successful candidate will coordinate with other professionals to assist in assessing family needs and make referrals to community resources. Incumbents will provide services to persons from a variety of social and economic backgrounds (a variety of ages, cultures, etc.). Public Health programs include California Children’s Services, the Child Health and Disability Program, General Public Health Nursing Field Services, Maternal, Child, and Adolescent Health programs, Communicable Diseases, Immunizations, and Tuberculosis programs. Candidates should have the ability to work with diverse populations in focusing on health promotion and education. The successful candidate must have the ability to maintain effective working relationships with the public, patients, staff, and other branches and agencies. The Tulare County Child Welfare Services (CWS) Division has a Public Health Nurse team that works under the direction of the Health Care Program for Children in Foster Care (HCPCFC). The HCPCFC, PHN, collaborates with business partners to identify and address health obstacles and barriers faced by foster youth. The goals and objectives of the HCPCFC PHN are to promote and improve access to comprehensive preventative health and specialty services for children in foster care. CWS public health nurses consult and collaborate with social workers to ensure the medical, dental, mental, and developmental needs of children and youth in foster care.

Requirements

  • Bachelor's degree in nursing
  • California State Public Health Nursing certificate
  • Registered Nurse license issued by the California Board of Registered Nursing
  • Two years of community nursing experience (for Level II candidates)

Responsibilities

  • Teach and counsel individuals, families, and groups regarding specific diseases or health conditions as well as health maintenance, promotion, and rehabilitation techniques.
  • Assist in the development and planning of public health programs.
