This Public Health Nurse position provides public health nursing services in the Health Protection Division, specifically in the Infectious Disease and Tuberculosis Control Program areas, which includes case, contact and outbreak management of communicable diseases.

Requirements

  • A Bachelor of Science in Nursing degree which includes preparation in Public Health Nursing from a recognized University.
  • Current registration with the College of Nurses of Ontario in good standing is required.
  • Knowledge of the principles of infectious diseases, tuberculosis control and immunizations.
  • Knowledge of Regional and Departmental policies and procedures, the Ministry of Health and Long Term Care Standards, Ontario Public Health Standards, Health Protection and Promotion Act, Health Care Consent Act, Personal Health Information Protection of Privacy Act, Municipal Freedom of Information and Protection of Privacy Act, Nursing Act, Child and Family Services Act, and other related legislation and professional guidelines and standards.
  • Skill in project management, strategy development, and report writing.
  • Critical thinking and innovative problem-solving skills.
  • Leadership, facilitation and team building skills.
  • Ability to model, support, and serve as a preceptor for professional and skills development.
  • Skill in priority setting, organization, and self-directed learning, with minimal supervision.
  • Ability to use and maintain electronic documentation for various purposes.
  • Ability to participate as an effective team member within an interdisciplinary health setting with community members of various backgrounds and cultures.
  • Ability to travel to various locations within, and occasionally outside Halton Region.
  • Excellent writing and communication skills, interpersonal skills, and skill in critical appraisal of research studies and knowledge of health promotion is essential.
  • Proficient and familiar with iPHIS.

Nice To Haves

  • Two years of public health experience is preferred.
  • 6-months of recent experience in counseling, educating, program planning, and implementation related to communicable disease management and investigation is preferred.
  • Familiarity with other software (e.g. M365, iPHIS, Halton Salesforce, Siebel) is an asset.

Responsibilities

  • Provides case management, health counselling, consultation, and education and resources to targeted individuals, groups, and stakeholders.
  • Completes screening and intake assessments.
  • Develops and implements educational initiatives on a variety of infectious diseases, vaccine preventable diseases (VPD) and infection prevention and control issues for agencies, institutions and the community at large.
  • Investigates reports of infectious diseases and participates in outbreak management, including respiratory outbreaks in institutions.
  • Participates in activities related to the management of emergencies of public health importance and participates in after-hours services.
  • Performs Direct/Virtual Observed Therapy (DOT/VDOT) services as required.
  • Participates in project and program planning, continuous quality improvement strategies, development, implementation, and evaluation.
  • Responsible for utilizing an electronic database as part of case management systems.
  • Acts as a preceptor, which includes assigning, monitoring, and coordinating learning assignments for nursing students, volunteers and onboarding of new staff.
  • Performs other duties, as assigned.
