About The Position
As a PT Social Worker, you’ll partner closely with the SW/CM to drive strong resident outcomes by supporting each person’s physical, emotional, and psychosocial well-being. This role exists to keep transitions smooth, communication clear, and resident rights front and center—every single day. Key responsibilities include assisting with discharge planning, coordinating communication between residents, families/representatives, and the interdisciplinary team, and advocating for resident rights in line with Fellowship standards.
Requirements
- Bachelor’s degree in Social Work
- Minimum two years’ experience in geriatric care
- Excellent interpersonal/public relations skills
- Strong organization skills
- Ability to counsel individuals in crisis
- Knowledge of senior community resources
- Intermediate computer skills
- Working knowledge of the MDS
- Understanding of Medicare/MA/managed care/private pay basics
- Understanding of medical terminology
