Requirements

Bachelor’s degree in Social Work
Minimum two years’ experience in geriatric care
Excellent interpersonal/public relations skills; strong organization
Ability to counsel individuals in crisis; knowledge of senior community resources
Intermediate computer skills; working knowledge of the MDS
Understanding of Medicare/MA/managed care/private pay basics; medical terminology

Responsibilities

Partner closely with the SW/CM to drive strong resident outcomes by supporting each person’s physical, emotional, and psychosocial well-being.
Keep transitions smooth, communication clear, and resident rights front and center.
Assist with discharge planning.
Coordinate communication between residents, families/representatives, and the interdisciplinary team.
Advocate for resident rights in line with Fellowship standards.
Connect with residents and family members.
Participate in quick huddles with nursing and other partners to keep everyone aligned.
Balance focused conversations with timely follow-ups.
Keep details organized and documentation current.
Shift gears between office work and on-unit presence.
Stay visible, calm, and ready when emotions run high.
Help keep plans on track, questions answered, and the team moving forward with confidence.
