About The Position

Telos Health Systems is seeking a Psychotherapist (LMHC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities. Requirements

  • Seeking English–Spanish bilingual professionals for this location.
  • Active Florida license as one of the following: Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Licensed Psychologist - PsyD/Ph.D.
  • Master’s degree (LMHC/LMFT) or Doctoral degree (Psychologist) in a relevant mental health field.
  • Ability to work independently while collaborating effectively within a clinical team.
  • Strong communication and clinical documentation skills.
  • Per AHCA requirements, employment for this position requires a Level II background check.

  • Experience working with geriatric populations.
  • Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.

Responsibilities

  • Conduct comprehensive mental health assessments and diagnostic evaluations.
  • Provide individual psychotherapy and ongoing treatment services.
  • Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Coordinate care with facility staff, medical providers, and other healthcare professionals.
  • Adhere to all federal, state, and organizational standards, policies, and regulations.
