Requirements

  • Master's in Social Work required.
  • PA Licensed Social Worker required.
  • Act 31 Child Abuse Reporting

Responsibilities

  • Facilitate the development, implementation, and ongoing review of patient treatment and discharge plans through active participation in interdisciplinary team meetings.
  • Complete case management duties for assigned patients, including preparing all required documentation, discharge materials, and psychosocial assessments within established timeframes.
  • Initiate discharge planning at the time of admission and maintain consistent documentation of all discharge‑related activities in the patient record.
  • Demonstrate behaviors aligned with SH Service Excellence standards by greeting others warmly, treating everyone with dignity and respect, and serving as a positive ambassador for Susquehanna Health.
  • Adhere to all organizational and departmental policies and procedures while taking responsibility for personal professional development and education.
  • Commit to high‑quality, safe work practices; identify potential inefficiencies or safety risks; avoid shortcuts that compromise safety; and promote patient and family involvement in safety‑related processes.
  • Report unsafe conditions promptly and address safety concerns in a constructive, educational manner. Participate in initiatives aimed at reducing risk and improving patient outcomes.
  • Schedule and coordinate involuntary commitment hearings, including petitioning and providing testimony as required.
  • Communicate with third‑party payors to obtain pre‑authorization for inpatient, outpatient, and continuing care services.
  • Assess patient needs and collaborate with psychiatrists and physicians to determine appropriate levels of care and referrals.
  • Complete all required patient documentation in a timely manner, including treatment plans, progress notes, therapy notes, psychosocial histories, safety plans, discharge plans, and inquiry assessments.
  • Provide therapeutic services including group psychotherapy, individual therapy, and family therapy.
  • Empower patients to participate actively in treatment decisions and leverage their strengths, abilities, and support systems to enhance quality of life.
  • Serve as a liaison to ensure coordinated communication among patients, families, referral sources, continuing care providers, and the treatment team regarding treatment progress and discharge planning.
