About The Position

The Nurse, Psychiatry provides patient-centered, trauma-informed support for patients with mental illness and co-occurring substance abuse/addiction disorders. They are a patient advocate: coordinates care and links patients to needed services; assess patient’s personal, medical, social, emotional and environmental situation; provides motivational support in treatment and therapy.

Requirements

  • Active Illinois Registered Nurse License and CPR certification required prior to hire date.
  • Excellent verbal and written communication skills as well as Compassionate listening skills.
  • Ability to read and interpret documents.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.

Nice To Haves

  • BSN preferred.
  • RN with a Bachelors in a related field also desirable.
  • Preference given to persons who have one or more years of experience in behavioral health nursing, social services and/or other related populations.
  • Community Health Nursing: experience in a community health setting is desirable.

Responsibilities

  • Assists the health center manager and provider with daily flow of patients in the health center, such as walk-in/phone triage, non-urgent patient calls, medication management and refills
  • Performs patient assessment including physical and psychosocial status; collaborates with provider for treatment plan.
  • Assesses health literacy of patients and families, providing appropriate, ongoing support and well-being.
  • Coordinates the delivery and support of health care plan to patients, family members, guardians and appropriate care-team members.
  • Evaluates patient’s response to interventions and collaborates with care team to revise goals, objectives and interventions appropriate to changes in the patient’s status. Coordinates transitions in care. Works with behavioral health team providing oversight in the coordination of referrals and reports.
  • Communicates and collaborates clearly and effectively with program operations to enhance patient centered care as needed.
  • Completes all documentation accurately thoroughly and within deadline. Participates in development of quality targets and measures and gathers reports and data.
  • Supports Medical Assistants with training, ongoing development and clinical support, as needed.
  • Performs nurse visits for medication management, patient education, vaccines and/or other assessments and interventions treatments as needed.
  • Performs other duties in the clinic per competency and/or as assigned, some examples include: Group education, Point of care testing, Medication Safety Surveillance, Annual staff competency assistance, Walk-in and phone triage, Assistance with overall health center nurse coverage, as directed by the manager.
