This role involves providing ongoing rehabilitation support through outreach, telehealth, and face-to-face interactions to help individuals identify recovery barriers, strengthen supports, and develop practical skills for independence and community integration. The specialist will assist individuals in achieving treatment, wellness, and recovery goals through skill-building, encouraging engagement in behavioral health services, healthy lifestyle practices, and self-advocacy. Serving as a key member of the Open Access team, the role includes engaging individuals at service entry, conducting initial needs assessments, and ensuring timely connection to appropriate supports. Additionally, the specialist will help individuals achieve educational goals by exploring opportunities and facilitating referrals, and support vocational and employment goals through guidance and connections to relevant services. Coordination of community linkages and progress monitoring, along with active participation in multidisciplinary case conferences and treatment team meetings, are also key aspects of the position.
