About The Position

This role involves providing ongoing rehabilitation support through outreach, telehealth, and face-to-face interactions to help individuals identify barriers to recovery, strengthen supports, and develop practical skills for independence and community integration. The specialist will assist individuals in achieving treatment, wellness, and recovery goals through skill-building, encouraging engagement in behavioral health services, healthy lifestyle practices, and self-advocacy. As a key member of the Open Access team, the specialist will engage individuals at the point of entry, conduct initial needs assessments, and ensure timely connection to appropriate services. Additionally, the role supports individuals in achieving educational and vocational/employment goals by exploring opportunities, facilitating referrals, providing guidance on career planning, resume development, interview preparation, and job readiness skills, and connecting them to relevant services and employment opportunities. The specialist will also coordinate community linkages, monitor progress, and participate in multidisciplinary case conferences and treatment team meetings to develop person-centered plans.

Requirements

  • Bachelors degree and 6 months of relevant experience required. Relevant experience includes previous work in a behavioral health setting in a patient facing role
  • Associates degree and 1 year of relevant experience required. Relevant experience includes previous work in a behavioral health setting in a patient facing role
  • High School Diploma/equivalent and 2 years of relevant experience required. Relevant experience includes previous work in a behavioral health setting in a patient facing role
  • Requires a valid NYS driver’s license and reliable transportation to travel locally on a regular basis.

Nice To Haves

  • Individuals with lived experience preferred.
  • Experience working within Electronic Health Record (EHR) systems and willingness to learn new software applications preferred.
  • Familiarity with reporting and entry level data analytics.

Responsibilities

  • Provide ongoing rehabilitation support through outreach calls, telehealth services, and face-to-face interactions, helping individuals identify barriers to recovery, strengthen existing supports, and develop practical skills that promote independence and community integration.
  • Assist individuals in achieving treatment, wellness, and recovery goals through skill-building interventions, encouraging engagement in behavioral health services, healthy lifestyle practices, self-advocacy, and other activities that support long-term success and overall well-being.
  • Serve as a key member of the Open Access team, engaging individuals at the point of entry into services, conducting initial needs assessments, and helping ensure timely connection to appropriate clinical, rehabilitation, and community-based supports.
  • Assist individuals in identifying and achieving educational goals, including exploring educational opportunities, facilitating referrals to training programs, colleges, GED services, and other learning resources, while providing ongoing encouragement and follow-up support.
  • Support individuals in exploring and achieving vocational and employment goals, providing guidance with career planning, resume development, interview preparation, and job readiness skills while connecting them to vocational rehabilitation services, workforce development programs, and meaningful employment opportunities within the community.
  • Coordinate community linkages and monitor progress, maintaining regular follow-up with individuals and service providers to ensure successful engagement with educational, vocational, and supportive services and to address barriers as they arise.
  • Participate actively in multidisciplinary case conferences and treatment team meetings, collaborating with providers, care managers, and community partners to develop person-centered plans that support recovery, independence, and overall well-being.
