About The Position

Working in behavioral health offers a level of purpose few careers can match. As a Psychiatric Rehabilitation Services Coordinator (PRSC), you will support adults on their recovery journey, helping them build skills, regain confidence, and move toward greater independence. Every day, you’ll see progress that matters. This role is part of a collaborative care environment within a 200-bed skilled nursing facility dedicated to providing high-quality care to older adults and the behavioral health population. The facility focuses on safe transitions of care, individualized treatment plans, and meaningful outcomes for each resident, with a clinical care approach that is resident-centered and team-driven.

Requirements

• Bachelor’s Degree in a human services field (including but not limited to sociology, special education, rehabilitation counseling or psychology)
• A minimum of 1 year of supervised experience in mental health or human services
• Prior experience as a PRSC or mental health professional in a licensed nursing home, intermediate care facility, or specialized mental health rehab facility is strongly preferred
• Experience with Point Click Care (PCC) is ideal, but not required

Responsibilities

• Provide group and individual psychiatric rehabilitation services
• Deliver case management support to adults with a history of psychiatric hospitalizations
• Help residents build daily living, coping, and social skills
• Develop and carry out therapeutic interventions focused on long-term stability
• Collaborate with an interdisciplinary care team to support individualized recovery plans
• Document services and progress according to program and regulatory requirements
