About The Position

The Psychiatric Rehabilitation Navigator provides person-centered case management and psychiatric rehabilitation services to adults experiencing a Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD). This role supports participants in achieving recovery, independence, and community integration by coordinating behavioral health, medical, social, vocational, educational, and community resources. The position promotes Illness Management and Recovery (IMR) principles while ensuring timely documentation, collaboration with interdisciplinary teams, and compliance with Medicaid, state, and agency requirements.

Requirements

  • Bachelor's degree from an accredited college or university in a human services or related field and one year of experience working/volunteering with people who have mental health conditions that have interfered with their quality of life OR High School Diploma/GED and three years of experience working/volunteering with people who have mental health conditions that have interfered with their quality of life
  • Valid New Mexico Driver's License, a record of safe driving practices (ie, a record that does not reflect unsafe driving practices), and current automobile insurance.
  • Ability to meet Medicaid and agency documentation standards.
  • Strong written, verbal, and interpersonal communication skills.

Nice To Haves

  • Experience with psychiatric rehabilitation, case management, or community behavioral health services.
  • Knowledge of New Mexico behavioral health systems and community resources.
  • Knowledge of serious mental illness, recovery principles, psychiatric rehabilitation, service planning, community resources, crisis intervention, benefits systems, behavioral health documentation, and Medicaid requirements.
  • Ability to build therapeutic relationships, coordinate services across systems, work independently and as part of an interdisciplinary team, maintain confidentiality, and effectively use electronic health records and Microsoft Office.

Responsibilities

  • Provide care navigation before admission, during services, and through discharge planning when appropriate.
  • Coordinate and provide transportation to and from the program four to five days a week in a safe and confidential manner. This includes transportation to external agencies as needed for the benefit of group activities consistent with the IMR model
  • Assist in the delivery of group based psychiatric rehabilitation interventions that promote recovery, independence, daily living skills, wellness, and community integration.
  • Assist in monitoring participant progress, health, safety, and engagement, modifying services as needs change.
  • Maintain regular communication with participants for the purposes of engagement
  • Coordinate crisis intervention services and communicate urgent clinical concerns as needed
  • Maintain accurate, complete, and timely documentation to meet Medicaid, agency, and regulatory requirements.
  • Participate in multidisciplinary meetings, supervision, quality improvement activities, and required training.
  • Maintain CPR/First Aid certification and perform other duties assigned in support of Psychiatric Rehabilitation Services.
